Once you have scheduled your initial appointment, please complete the required registration online via our secure patient portal. Completing the registration prior to your visit will help expedite the check in process and avoid delays in the office.
Additional items required of new patients at the time of the visit include:
- Photo ID
- Insurance card
- Referral form from your primary care physician, if one is required
- Current glasses
- Current list of medications with dosages
- Pharmacy address and phone number
- Translator, if necessary
Once the form has been downloaded to your computer, you may use Adobe Acrobat Reader to view form before you print and sign it.
INSURANCE & PAYMENT
At Ideal EyeCare, we accept Medicare and most major insurance plans. Please contact your insurance company or you may call our office at (702) 896-2020 to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.
Prescriptions and refills for established patients of Ideal EyeCare are processed only during our regular office hours and will not be issued during evenings, weekends, or holidays. It is helpful to anticipate your refill needs to avoid interruption in your medication therapy. You may request refills via our secure patient portal or you may have your pharmacy send an electronic request for refills. Please allow 24 to 48 hours for refill approvals.
